Residents also have the option to book their appointment by phone at 1-833-943-3900.
Residents need to bring identification on the day of their appointment. Clinic team members need this information to confirm they are part of an eligible priority group. For occupational groups like heath care workers, this can be a workplace identification badge, proof of employment or a letter from their employer.
Clinic staff take vaccine prioritization seriously. Providing false information may result in cancellation of the appointment.
Homebound residents who have physical, social or mental barriers that inhibits them from visiting a clinic, pharmacy, or primary care to receive the COVID-19 vaccination outside their home will be personally contacted via mail and/or phone call by Toronto Paramedic Services to arrange a vaccination appointment. You will only be contacted if you are a LHIN Home and Community Care client. If you are not a LHIN client, please contact your Primary Care Provider.
